On Line Complaint Form

You can use this form (See href="#Note">NOTE below) to report
allegations of misconduct in state government. When filling out this
form, please provide as much information as possible, which may
include the following:

  • Who is engaging in misconduct?
  • Which state agency is involved?
  • What wrongdoing occurred?
  • When did it happen?
  • Are there witnesses to the misconduct that we can contact?
  • What laws or agency regulations have been violated?

relates to matters within our jurisdiction, please call our hotline,

If you request confidentiality, we will make every effort to keep your identity confidential consistent with applicable New York State and federal laws, rules and regulations, and the provisions of our privacy policy. Our office does accept anonymous complaints.

ATTACHMENTS:   If you wish to send us a document
attachment to accompany your complaint, don't use this form but contact us instead by sending an e-mail with the attachment to inspector.general@ig.state.ny.us
Only documents in standard word processing or text formats (i.e.,
Microsoft Word (.doc), WordPerfect (.wpd), generic word processing (.rtf),
plain text (.txt) or Adobe Acrobat (.pdf) documents) not exceeding 1 MB in
size are acceptable.  Other document formats or oversized attachments
may be incompatible with and rejected by our e-mail system.
